In this section, you will find all of the information you need to make an informed decision about your employee benefits options. Bring your completed benefits packet and all specified information with you on your first day of orientation. Please complete all forms in black ink.

Please refer to your appointment letter for your employee status: regular, contingent II (.75 FTE – 1.00 FTE), contingent II (.50 FTE - .74 FTE), or adjunct faculty.
